Estimated Contagious Equine Metritis (CEM) Costs
Average base cost for uncomplicated CEM veterinary fees performed at an approved facility other than The Piedmont Equine Practice (PEP) – $2,100-$2,400. This EXCLUDES boarding fees, state veterinarian fees, and any additional supplies or services that may be required (outside of the normal CEM process).

State Veterinarian Fees
State veterinarian fees as of 2025 are $500-$600 and cover the cost of the state veterinarian meeting the trailer on mare’s initial arrival to the farm and meeting the CEM-certified veterinarian on the first day of the wash and pack treatment series (required in all imports). The state veterinarian fees are not billed through PEP, but rather directly from the State Veterinarian’s office.

Additional Recommended Services
Coggins, CORE EQ vaccine (EWT, WNV, and Rabies), Flu/Rhino vaccine, and Potomac Horse Fever vaccine if mare is not current for these things. Charge for these services not included in overall CEM price; contact PEP for current Coggins and vaccination pricing.
As the owner and/or financially responsible individual for the equine presenting for CEM services, I understand that complications could arise that may incur fees above and beyond the listed estimates, and those services will be discussed with me prior to them being performed. Examples of complications that would increase the cost of services may be a horse spiking a fever, a horse developing a condition other than CEM needing antibiotic therapy, or a positive culture result.
I understand that services related to CEM quarantine and performed by The Piedmont Equine Practice must be paid in full 24 hours prior to the horse being released from quarantine by the state veterinarian.
